Wanted concrete floor prepper and finisher

I'm looking for someone who knows how to make a form and prepare an old existing concrete floor in my garage for pouring fresh concrete. You would need to have experience and possibly a concrete vibrator. The area is about 25x40' need to raise 4 inches. Looking to pay $1000 . Thanks
