Reporting to the Director of Care or designated clinical leadership team member, the Personal Support Assistant helps with select nursing and personal care tasks in accordance with residents plan of care and, which are within their knowledge and skill, while adhering to regulatory and Reveras standards, policies and procedures. Key Responsibilities
- To support and provide one on one care as needed;
- To provides day to day services and support to individual residents to offer a warm and safe environment, ensuring comfort and well-being of residents are met;
- To support with safe lifts and transfers of residents;
- To guide residents to and from their rooms, recreational activities, restorative care office and dining room as required while contributing to a positive resident experience;
- To act as a resident companion and provide comfort and support to residents as needed;
- To assist with feeding while adhering to safe food handling techniques throughout all aspects of food service;
- To assist in completing general duties as directed by the care staff including, but not limited to organizing closets, restocking shelves, making of beds, support residents with high needs in choosing their clothing and dressing;
- To participate on an as needed basis in interdisciplinary meetings for assessing, planning, implementing and evaluating resident care and service needs;
- To work cooperatively with peers and supervisors using the Quality Improvement/CQI process ensuring timely follow-ups;
- To assist in maintaining a safe and clean environment for residents utilizing infection control principles;
- To communicate with supervisor about problems related to resident care and/or safety;
- To handle tactfully and courteously with residents, residents families and reports any concerns/complaints to the supervisor;
- To participate in the orientation process for residents, students, volunteers and care staff;
- To actively participate in ongoing educational opportunities and transmits information to the rest of the care staff;
- To maintain resident/resident information confidentiality;
- To uphold and promote the organizations values and philosophy relating particularly to ethics, morality, and integrity as set out in Reveras Code of Conduct;
- To complete all other tasks as assigned.
- High School Diploma;
- Working towards completing Personal Support Worker program (or provincial equivalent certificate program) or any other healthcare program nice to have;
- Volunteer/Working experience with seniors in the community health services setting nice to have;
- Ability to model warm, caring and empathetic communication style when connecting with residents, families and peers;
- Strong and personable communication skills;
- Demonstrated desire to work in a service driven environment and through customer focus skillset.

The Opportunity
Ready to make a difference in the lives of seniors? Were looking for an experienced Registered Practical Nurse to join our team of dedicated health professionals. Reporting to the Director of Care, you will play a key role in our patient-centred, goal-directed approach to care. From evaluating clinical care plans to administering medication and other treatment, your role is as important as it is fulfilling.
A typical day might include:
Fostering relationships with residents and families to ensure plan of care is resident focused and carried out;
Evaluating a residents plan of care, ensuring the assessment is up to date and accurately documented;
Processing and administering medication in accordance with regulatory requirements;
Collaborating with and advising Personal Support Workers and staff on best practices related to the delivery of our health-care programs and policies;
Supporting the arrival, transfer or departure of residents;
Responding to emerging health conditions and medical emergencies, including the delivery of first-aid care.
What YOU bring
Current registration as a Registered Practical Nurse in your province of work
Experience working with seniors and alongside Personal Support Workers, as well as education tied to gerontology, rehabilitation and long term care
Current basic Cardiac Life Support and First Aid certification
Proficient assessment, documentation and communication skills
Proficiency with technology including: Electronic Health Records, Electronic Medication Administration Records, Online Applications
Current (within 6 months) Vulnerable Position/Sector Check (VSC)

Reporting to the Director of Care (DOC) or Director of Health and Wellness (DHW), the Registered Practical Nurse (RPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care. Key Responsibilities - To work as a member of the inter professional team as described by Revera's Professional Practice Model applying the person centered approach to care;- To complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required;- To implement and evaluate the effectiveness of the care/service plans and make changes as required;- To assist with scheduling of resident's care conferences and participate in conferences where applicable;- To work closely with DOC/DHW to facilitate resident's move in, move out and transfers;- To provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Revera Resident Care standards and policies and procedures;- To process the orders and safe keep drugs and narcotics and administer in compliance with all regulatory requirements, scope of practice and Revera policies and procedures;- To facilitate and provide resident/family education;- To plan, coordinate, manage & evaluate the resident's care plan working with the resident/substitute decision maker, physicians, nursing & interdisciplinary team and contracted service providers;- To coordinate nursing team's resident care assignments to ensure efficient and timely delivery of services to residents;- To respond to and manage significant changes in resident's condition and or medical emergencies; provide emergency first aid;- To teach, delegate to, and direct unregulated care providers;- To respond to resident/family concerns and ensure appropriate action taken and or concern communicated to DOC/DHW/Executive Director and other departments;- To follow and assist the DOC/DHW in the coordination, implementation and monitoring of Infection Prevention and Control , Health and Safety and Clinical Programs and Quality/Safety Plans for the Nursing Department;- To uphold and promote the organization's values and philosophy relating particularly to ethics, morality, and integrity as set out in Revera's Code of Conduct;- To complete all other duties as assigned. Qualifications - Current registration as a Registered Practical Nurse in applicable Province/State;- Experience working in a senior's environment and with unregulated care providers preferred;- Education and experience in gerontology, rehabilitation, community/complex/long term care an asset;- Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills;- Must have demonstrated sound clinical knowledge;- Current Basic Cardiac Life Support and First Aid certification required;- Computer proficiency and experience with Microsoft office applications.- Current (within 6 months) Vulnerable Position/Sector Screening (VPS).
