Reporting to the Director of Care or designated clinical leadership team member, the Personal Support Assistant helps with select nursing and personal care tasks in accordance with residents plan of care and, which are within their knowledge and skill, while adhering to regulatory and Reveras standards, policies and procedures.
Key Responsibilities- To support and provide one on one care as needed;
- To provides day to day services and support to individual residents to offer a warm and safe environment, ensuring comfort and well-being of residents are met;
- To support with safe lifts and transfers of residents;
- To guide residents to and from their rooms, recreational activities, restorative care office and dining room as required while contributing to a positive resident experience;
- To act as a resident companion and provide comfort and support to residents as needed;
- To assist with feeding while adhering to safe food handling techniques throughout all aspects of food service;
- To assist in completing general duties as directed by the care staff including, but not limited to organizing closets, restocking shelves, making of beds, support residents with high needs in choosing their clothing and dressing;
- To participate on an as needed basis in interdisciplinary meetings for assessing, planning, implementing and evaluating resident care and service needs;
- To work cooperatively with peers and supervisors using the Quality Improvement/CQI process ensuring timely follow-ups;
- To assist in maintaining a safe and clean environment for residents utilizing infection control principles;
- To communicate with supervisor about problems related to resident care and/or safety;
- To handle tactfully and courteously with residents, residents families and reports any concerns/complaints to the supervisor;
- To participate in the orientation process for residents, students, volunteers and care staff;
- To actively participate in ongoing educational opportunities and transmits information to the rest of the care staff;
- To maintain resident/resident information confidentiality;
- To uphold and promote the organizations values and philosophy relating particularly to ethics, morality, and integrity as set out in Reveras Code of Conduct;
- To complete all other tasks as assigned.
Qualifications- High School Diploma;
- Working towards completing Personal Support Worker program (or provincial equivalent certificate program) or any other healthcare program nice to have;
- Volunteer/Working experience with seniors in the community health services setting nice to have;
- Ability to model warm, caring and empathetic communication style when connecting with residents, families and peers;
- Strong and personable communication skills;
- Demonstrated desire to work in a service driven environment and through customer focus skillset.

Pathways to Independence is seeking an experienced Clinical Therapist to join our Clinical Services Team. Reporting to the Director of Clinical Services, the Clinical Therapist (CT) works as an integral part of an interdisciplinary team to promote client-centered care through individual, couple and group interventions aimed to optimize health, emotional and social outcomes. With a trauma-informed lens, and an emphasis on the Social Determinants of Health, the incumbent will conduct comprehensive assessments of complex adults and will employ a variety of treatment modalities to promote adaptive functioning, inclusion, and psychological resilience. Pathways to Independence is seeking an experienced Clinical Therapist to join our Clinical Services Team. Reporting to the Director of Clinical Services, the Clinical Therapist (CT) works as an integral part of an interdisciplinary team to promote client-centered care through individual, couple and group interventions aimed to optimize health, emotional and social outcomes. With a trauma-informed lens, and an emphasis on the Social Determinants of Health, the incumbent will conduct comprehensive assessments of complex adults and will employ a variety of treatment modalities to promote adaptive functioning, inclusion, and psychological resilience. The Clinical Therapist will develop and deliver training and education to employees; evaluate individual and program outcomes; and collaborate with a variety of service providers to improve clinical trajectories. The successful incumbent will be responsible for:* Offering a range of professional social work and therapeutic interventions consistent with one’s scope of practice.* Conduct comprehensive biopsychosocial assessments, develop individualized treatment plans with measurable goals and outcomes.* Provide individual, dyad or group counselling and/or psychotherapy (if authorized under legislation) to referred individuals.* Network with community-based service providers, to improve program outcomes, streamline workflows and strengthen partnerships, engage in advocacy with community resources to address discrepancies in outcomes and provide more comprehensive supports for people with complex needs.* Supervise staff on implementation of clinical therapeutic strategies, plans, and protocols, and provide feedback to employees, and performance information to employee’s manager.* Provide education/workshops to employees and participate in ongoing continuing education activities. * Develop, implement and evaluate new policies and procedures specific to therapeutic interventions.* Lead team communications with individuals and their families when complex dynamics are adversely impacting the provision of services.* Produce timely, comprehensive documentation in accordance with all agency, departmental and professional standards and guidelines.Experience and education:* Bachelor’s or Master’s degree in social work from an accredited university.* Registration with the Ontario College of Social Workers and Social Service Workers, or eligibility to become registered.* Demonstrated knowledge and 3-5 years’ experience working with adults with a developmental and/or intellectual disability, dual diagnosis, acquired brain injury and/or related mental health/addiction issues.* Understanding of diagnostic criteria and clinical experience in assessment formulation and report writing.* Knowledge of trauma informed care and the strengths-based approach.* Knowledge of a variety of treatment modalities and social work assessment tools (e.g. genograms, ecomaps, SWOT analysis).* Working knowledge of pertinent legislation.* Additional training specific to indigenous/cultural safety is an asset.* Experience with court, justice and/or child welfare systems is an asset.* Exceptional oral and written communication skills, including the ability to network with community-based service providers, demonstrated computer literacy.The successful candidate must be able to perform the job duties as outlined in the job description.
