Pathways to Independence is seeking an experienced Clinical Therapist to join our Clinical Services Team. Reporting to the Director of Clinical Services, the Clinical Therapist (CT) works as an integral part of an interdisciplinary team to promote client-centered care through individual, couple and group interventions aimed to optimize health, emotional and social outcomes. With a trauma-informed lens, and an emphasis on the Social Determinants of Health, the incumbent will conduct comprehensive assessments of complex adults and will employ a variety of treatment modalities to promote adaptive functioning, inclusion, and psychological resilience. The Clinical Therapist will develop and deliver training and education to employees; evaluate individual and program outcomes; and collaborate with a variety of service providers to improve clinical trajectories. The successful incumbent will be responsible for:* Offering a range of professional social work and therapeutic interventions consistent with one’s scope of practice.* Conduct comprehensive biopsychosocial assessments, develop individualized treatment plans with measurable goals and outcomes.* Provide individual, dyad or group counselling and/or psychotherapy (if authorized under legislation) to referred individuals.* Network with community-based service providers, to improve program outcomes, streamline workflows and strengthen partnerships, engage in advocacy with community resources to address discrepancies in outcomes and provide more comprehensive supports for people with complex needs.* Supervise staff on implementation of clinical therapeutic strategies, plans, and protocols, and provide feedback to employees, and performance information to employee’s manager.* Provide education/workshops to employees and participate in ongoing continuing education activities. * Develop, implement and evaluate new policies and procedures specific to therapeutic interventions.* Lead team communications with individuals and their families when complex dynamics are adversely impacting the provision of services.* Produce timely, comprehensive documentation in accordance with all agency, departmental and professional standards and guidelines.Experience and education:* Bachelor’s or Master’s degree in social work from an accredited university.* Registration with the Ontario College of Social Workers and Social Service Workers, or eligibility to become registered.* Demonstrated knowledge and 3-5 years’ experience working with adults with a developmental and/or intellectual disability, dual diagnosis, acquired brain injury and/or related mental health/addiction issues.* Understanding of diagnostic criteria and clinical experience in assessment formulation and report writing.* Knowledge of trauma informed care and the strengths-based approach.* Knowledge of a variety of treatment modalities and social work assessment tools (e.g. genograms, ecomaps, SWOT analysis).* Working knowledge of pertinent legislation.* Additional training specific to indigenous/cultural safety is an asset.* Experience with court, justice and/or child welfare systems is an asset.* Exceptional oral and written communication skills, including the ability to network with community-based service providers, demonstrated computer literacy.The successful candidate must be able to perform the job duties as outlined in the job description. We are looking for an occasional Caregiver with a Vehicle to drop-off & pickup my father from dialysis, (Lakeridge Health Oshawa).
Tuesdays/Thursdays: 3pm-5pm & 9pm-11pm (Ocassionally)
Saturdays (Ocassionally) : 3pm-5pm & 9pm-11pm
A Certified PSW is not necessary. But we do require the aide to have experience and a general understanding for seniors. Aide Must be located in Durham.

Seeking reliable, energic nurse (RPN or RN) to work with a child who is ventilator and wheelchair dependant, and requires 24/7 airway monitoring.
Shifts: every other Saturday (ie 2/month) and 2 evenings/week. More shifts available if interested.
Pay Rate: starting at $33/hour (RPN rate)
During your shift you will be responsible for:
- managing respiratory care (including ventilator and suction machines),
- managing trach care,
- g-tube feeds,
- managing gtube care,
- administering medications,
- using a cough assist (an artificial cough assistant device),
- Range of motion, physio,
- experience with Hoyer lift,
- activities of daily living (ADLs), and
- General care as needed.
Requirements:
- Current unrestricted licence to practice through the College of Nurses of Ontario
- A valid driver’s licence and vehicle
- A clear vulnerable sector check
- Current CPR certification
- Proof of liability insurance (if you're registered through CNO you have it already)
You must be able to comfortably lift up to 50lbs and/or using a lift and comfortable using technology (computer, IPads, etc) and you must love working with children.

Reporting to the Director of Care, your expertise and careful administration of resident care will help ensure we retain our reputation for delivering the best in person-centred care for seniors across Canada.
This role includes:
Fostering relationships with residents and families to ensure plan of care is resident focused and carried out;
Managing, evaluating, and facilitating residents plan of care through collaboration with the resident, family members and the interdisciplinary team;
Participate in residents care conferences;
Collaborating with the Director of Care in facilitating resident move-in and transfers;
Ensure safe medication administration according to regulatory requirements and Reveras policies and procedures, including delegating medical administration to Unregulated Care Providers such as Personal Support Workers or Health Care Aides;
Complete Clinical Assessments that ensure appropriate resident care;
Liaise with interdisciplinary team and external care providers to facilitate best plan of care for each resident;
Lead and support Personal Support Workers and Health Care Aides in your home area to maximize resident care.
What YOU bring
Current registration as a Registered Nurse in applicable Province
Education and experience in gerontology, rehabilitation, community/complex/long term care an asset
Experience working with seniors and unregulated care providers
Strong assessment, documentation, communication and critical thinking skills
Strong written, oral and time-management skills, including demonstrated experience delegating tasks to Personal Support Workers / Health Care Aides
Proficiency with Technology including: Electronic Health Records, Electronic Medication Administration Records, Online Applications
Current Basic Cardiac Life Support and First Aid certification
Current (within 6 months) Vulnerable Position/Sector Check (VSC)
